Humber Meadows Long-Term Care Home (HMLTCH) is proud to be a pre-designate Best Practice Spotlight Organization (BPSO) through the Registered Nursing Association of Ontario (RNAO)

As a designated Best Practice Spotlight Organization (BPSO), our home is proud to lead with evidence-based care that enhances the health, safety, and well-being of every resident. Through our partnership with the Registered Nurses’ Association of Ontario (RNAO), we implement internationally recognized best practice guidelines that strengthen resident experience, support our staff, and elevate the quality of care across our home.

We are are excited to be the first long-term care home in Canada to achieve pre-designation status prior to opening, beginning our journey on April 1, 2023.

Being a BPSO reflects our dedication to continuous learning, innovation, and collaboration. This page highlights the initiatives, achievements, and ongoing work our team has undertaken to ensure residents and families benefit from the highest standards in long-term care.

Our BPSO Leads

Leading us through our journey to become a Best Practice Spotlight Organization is Ashley, Director of Resident Programs, and Olivia, RAI Coordinator.

Ashley supports our overall BPSO work, palliative care and people-centered care work.

Olivia supports our BPSO work, pain prevention and management work along with is instrumental in our clinical pathway roll outs and management.

 

Since implementing best practice guidelines, our home has celebrated many successes! Some of these accomplishments and successes include:

  • Strengthened an Interdisciplinary Culture: Built a robust, collaborative approach supported by more than 100 staff members in becoming  trained BPSO champions across departments.

  • Showcased Our Work Provincially: Presented our interdisciplinary approach to palliative care at the BPSO Symposium, sharing strategies and outcomes with sector partners.

  • Elevating Resident Voice: Our Director of Resident Programs, together with a resident, spoke at the April 2025 BPSO Launch to highlight the essential role of residents’ perspectives in shaping every home’s BPSO journey. This accomplishment also marked the first time a resident spoke at a BPSO launch in the history of the program.

  • Sharing Knowledge Internationally: Welcomed International BPSOs to our home to exchange insights, showcase our implementation work, and learn from global best practices.

  • Celebrating Our Champions: Introduced a “Champion Wall” to recognize and honour the staff who lead and support best practice work every day. This wall was recently upgraded to display not only our champions name but highlighting whether they are a people-centred care, palliative or pain champion.

  • Enhancing Visibility Across the Home: Installed BPSO information boards in our main hallway to explain what it means to be a BPSO and outline our progress toward each guideline. These boards are located so all residents, visitors and staff can understand what being a BPSO means and all the work we’ve been doing to make the home the best it can be.

Since implementing the People-Centred Care best practice guidelines, our home has celebrated many successes! Some of these accomplishments and successes include:

  • Getting to Know Our Residents: Utilize All About Me packages and posters to build stronger relationships with each resident, gaining insight into their past experiences, preferences, and daily needs.

  • Rolling Out “Get to Know Me” Posters: Began placing personalized posters for each resident, allowing staff to build stronger and more inclusive connections beyond standard care interactions.

  • Supporting Transitions: Provide new residents and families with an Activity Package during admission, helping them adjust to the home and navigate their resident area, floor, and main floor spaces.

  • Investing in Staff Education: Partnered with the CLRI PSW Fund to provide staff with training in Living the Dementia Journey and Person-Centred Culture, enhancing learning opportunities and strengthening care for all residents.

  • Commitment to People-Centred Care: Committed to transitioning from Person- and Family-Centred Care to People-Centred Care guidelines, implemented across the home in 2025.

  • Demonstrating Commitment: Implemented a Person-Centered Care Pledge for staff, students, and volunteers, complemented by a mural in the home that symbolizes our dedication to residents and visitors.

  • Enhancing Family Engagement: Rolling out two specialized family portals—one focused on engagement and communication, and the other on health care updates—to keep families informed and connected.

  • Resident Feedback in Action: Actively involve the Residents’ Council in program feedback, especially within palliative care, ensuring resident perspectives shape care approaches.

  • Culturally Responsive Spaces: Reshaped our four Italian-designated home areas to include fully translated signage, Italian-based partwork, and European and Italian-themed murals, fostering a culturally inclusive environment.

 

Our People-Centered Care Lead – Naomi Kovach, BSO Lead

A Palliative Approach to Care in the Last 12 Months of Life | RNAO.ca

Since implementing the palliative care best practice guidelines, our home has celebrated many successes! Some of these accomplishments and successes include:

    • Enhanced Comfort and Support: Developed and introduced palliative care support carts equipped with meaningful comfort items for residents and families during end-of-life care.

    • Sector Leadership: Became the first new home to enroll in and successfully complete the CLRI Sustaining Palliative Care Project in 2024, demonstrating our commitment to leading best practices in the province.

    • Accessible Resources for All: Created comprehensive Palliative Care Support Binders containing policies, educational materials, and supportive documents for staff, residents, and families at every stage of the palliative journey.

    • Family-Centered Information: Developed a home-specific Palliative Care Brochure outlining available services, supports, and care options for residents receiving palliative or end-of-life care, ensuring families feel informed and supported from the start.

    • Building Teamwide Expertise: Provided introductory palliative care training to 150 interdisciplinary team members, strengthening knowledge, confidence, and consistency across the home.

    • Ongoing Professional Development: Joined the CLRI All-In Palliative Care: The Team Approach to LTC program to further enhance our palliative care practices and implementation strategies.

    • Comfort for Families: Made cots available for families to stay overnight during end-of-life care and ensured refreshments are accessible to support loved ones during this difficult time.

    • Honouring Residents with Compassion: Implementing a “Code Butterfly” initiative to honour residents after their passing—now expanded to include recognition and support for staff, co-residents, and visitors.
